How to Disenroll

If you decide you want to disenroll from your current plan, you can do so for any reason between November 15 and December 31 of every year.

Disenrollment means that you are ending your membership with Today's Health® HMO. Disenrollment can be voluntary (your choice) or involuntary (not your own choice).

To disenroll from our plan, you must submit a written request to the plan. Please mail your disenrollment request to:

There are also a few situations where you would be required to leave. For example:

  • If you move out of our geographic service area;
  • Or, if Today's Health no longer offers prescription drug coverage.

We are not allowed to ask you to leave the plan because of your health.

Until your prescription drug coverage with Today's Health ends, use our network pharmacies to fill your prescriptions.

If you leave Today's Health, it takes some time for your prescription drug coverage to end and your new prescription drug coverage to begin.

Enrollment is generally for the calendar year. In certain cases, such as if you move or enter an assisted living nursing home, you may be able to disenroll from your plan prior to the end of the year.

After you request to disenroll, Today's Health will let you know, in writing, the date your coverage ends.

While you are waiting for your membership to end, you are still a member and must continue to get your prescription drugs through Today's Health’s network pharmacies. In most cases, your prescriptions are covered only if they are filled at a network pharmacy, listed on our formulary and follow other plan rules.

Potential for Contract Termination

In the unlikely event your Plan Sponsor leaves the Medicare program, or no longer offers prescription drug coverage in the service area where you live, we will notify you in writing.

If this happens, your membership in Today's Health® will end, and you will need to enroll in another Medicare Prescription Drug Plan to continue your coverage. All of the benefits and rules of Today's Health® would continue until the effective date your membership in Today's Health® ends. You also would continue to receive your prescription drugs in the usual way through Today's Health® network pharmacies.

If Today's Health® leaves the Medicare program or your service area; you would need to select another plan. Your choices would include joining another Medicare Prescription Drug Plan or a Medicare Advantage Plan with prescription drug coverage (MA-PD), if these plans are available in your area and are accepting new members. As soon as Today's Health® has notified you in writing that we are leaving the Medicare program or the area where you live, you may enroll in another plan.

The Plan Sponsor is contracted with the Centers for Medicare & Medicaid Services (CMS), the government agency that runs Medicare. This contract is subject to renewal each year. However, the Plan Sponsor or CMS can decide to end the contract at any time. You will generally be notified 90 days in advance if this situation occurs. However, your advance notice may be as little as 30 days or fewer if CMS must end the contract in the middle of the year.
