Medication Therapy Management Program (MTMP)

We have a program that may help our members with special situations. For example, some members have several complex medical conditions or they may need to take many drugs at the same time, or they could have very high drug costs. The Medication Therapy Management Program can help you and your doctors get the most from your drugs. This program is not considered part of your benefit.

Who qualifies for MTMP?

You will be automatically enrolled in this program if you:

• take eight (8) or more drugs every day
• have three (3) or more long-term health conditions, such as
  • Diabetes
  • Chronic Heart Failure
  • Cardiovascular Disorders
  • Osteoporosis
  • Asthma
  • Alzheimer’s Disease
  • Depression
  • Schizophrenia
  • Bipolar Disorder
• and spend more than $3,100.20 a year on your drugs

If you are selected, this program is free to you.

How will I know if I’m selected?

Once you are selected, you will receive an invitation to have a personal phone call with one of our network pharmacists to review all of the drugs, vitamins and herbal products you are taking.

How does it work?

This program works with you and your doctor to make sure the drugs you are taking are safe and working for you. Specially trained pharmacists will review your drugs. They will let your doctor know ways to help you better manage your drug therapy. You will be invited to review all of your drugs and any other vitamins or herbal products you take with one of our network pharmacists who can answer any questions you have.
