Understanding Your Coverage

The Tribute^® HMO SNP Medicare Advantage Plan is an affordable way to manage your healthcare costs associated with living in a nursing home or you live in the community or in an assisted living facility and require the same level of care as someone in a nursing home.

The Medicare Advantage plans from Universal American’s family of healthcare companies all offer coverage for visits to doctors and specialists, but each type of plan works a little differently.

With a Tribute Medicare Advantage plan, you may go to any doctor or hospital that is included in our network. If you choose a doctor or medical facility that is not included*, you will be responsible for all costs – except in emergency situations. In addition, you will be asked to choose a primary care doctor. This doctor will coordinate all of your medical care, including choosing specialists when additional care is needed.

*You must use plan providers except in emergent or urgent care situations (or for out-of-area renal dialysis). If you obtain routine care from out-of-network providers neither Medicare nor Tribute will be responsible for the costs.
