Tribute® HMO SNP Plans
Your Tribute
plan offers all the benefits and services of Original Medicare, including Medicare Part D Prescription Drug coverage, as well as Medicaid for those who are eligible. It also offers additional services at no extra cost.
Additional Coordinated Care Services
As part of the plan, Tribute
brings the added service of a mid-level practitioner (nurse practitioner or physician assistant) to you. This mid-level practitioner helps with:
Assessment
- Conducts comprehensive assessments to determine your health status
- Visits each member routinely to ensure health status is maintained
Coordination
- Supports on-going communication between long term facilities, physicians and other network providers
- Expedites authorization of services that impact your medical care
Prevention
- Identifies potential risks to you and initiates early intervention
- Ensures timely and appropriate use of medical services
Through these activities, the mid-level practitioner serves as an extension of your Primary Care Physician to help maintain your health and well being. We ensure that each Tribute
member receives the attention they need, the care they require and the comfort they deserve... all at no additional cost.
