Ready to enroll?
The Tribute® HMO SNP plan is designed for people who meet specific enrollment criteria. If you're dual-eligible and living in a nursing home or you live in the community or in an assisted living facility and require the same level of care as someone in a nursing home, you may qualify for a Special Election Period (SEP) and enroll in a Tribute plan at any time.

Before you enroll
- Have your red, white and blue Medicare card with you as a reference.
- Have your checking account number and routing number available, if you plan to use automatic bank withdraw for your monthly premium payment. (Note: this is only if you plan to enroll through a paper enrollment application).
- Select a Primary Care Physician from the Provider Directory.
- Read the enrollment disclaimer information.

Medicare beneficiaries may enroll in Tribute through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.Medicare.gov (by clicking on this link you will be leaving our Web site).
Special Election Period (SEP)
Special Election Periods allow you to enroll in, or change plans outside of Medicare's standard enrollment periods. For example, if you move out of your Medicare Advantage plan’s service area, you have a Special Election Period to enroll in a plan that is available in your new location. If you move into, reside in, or move out of a nursing home you may also have a Special Election Period. If you have Medicare and Medicaid, you have a Special Election Period that allows you to change Part D drug plans at any time.
