Understanding Your Coverage

TexanPlus plans from Universal American’s family of healthcare companies all offer coverage for visits to doctors and specialists, but each type of plan works a little differently.

With TexanPlus plans you will choose a doctor from our broad network to coordinate your care. There is no limit on covered office visits, so you can see your doctor as often as you choose.* If you wish to change your doctor, also called your Primary Care Physician (PCP), you may do so at any time. Your PCP is your health advocate – he or she will send you to specialists or hospitals when you need additional care.

Please note that with an HMO plan, you must use providers in the network. You will be responsible for any fees or services provided by doctors or hospitals out of the network. Providers in the network can change at any time.

Disclaimer Information:

Medicare Complaint Form. For more information, visit Medicare.gov or Medicare's Ombudsman. (By clicking on these links you will be leaving our website).

TexanPlus® HMO is a Health plan with a Medicare contract.

Generations Healthcare HMO, TexanPlus® HMO, Today's Options® HMO, and Tribute® HMO SNP are Health plans with a Medicare contract.

* You must use plan providers except in emergency or urgent care situations (or for out-of-area renal dialysis). If you obtain routine care from out-of-network providers neither Medicare nor TexanPlus will be responsible for the costs.

The benefit information provided is a brief summary, not a complete description of benefits. For more information contact the plan.
Limitations, copayments, and restrictions may apply.
Benefits, formulary, pharmacy network, premium and/or copayment/co-insurance may change on January 1 of each year.

We also list pharmacies that are in our network but are outside Austin, Brazoria, Chambers, Fort Bend, Galveston, Hardin, Harris, Jefferson, Liberty, Montgomery, Orange, and Waller counties in Texas. Please contact TexanPlus at 1-866-230-2513, 8:00 a.m. to 8:00 p.m. in your local time zone, 7 days a week, for additional information.
