The Annual Election Period is over.  However, you may still qualify to enroll in a plan for coverage in 2010 if you meet certain qualifications.  Find out if you qualify.

If you are a Prescribrx member and want to change to a different Prescribrx plan, use our easy 2010 Change Notice Form instead of an enrollment form.

Before you enroll:

• Be sure you know the name of the plan you want to enroll in. If you’re not sure, use our plan finder.
• Have your red, white and blue Medicare card with you as a reference.
• Have your checking account number and routing number available, if you plan to automatically deduct your monthly premium from your checking account. (Note: this is only if you plan to enroll online or through a paper enrollment application).
• Read the enrollment disclaimer information.

Choose one of four convenient ways to enroll:

1. Enroll online by submitting our secure online form.
2. Call a PrescribaRx Customer Service Representative at 1-800-818-0007 8:00 a.m. to 8:00 p.m. in your local time zone (TTY users call 1-800-777-9083) every day, to enroll over the phone.
3. Download a printable enrollment form to mail or fax to us.

4. Schedule a call with a licensed insurance specialist and enroll using a paper application.

Part D Medicare Reminder:

• If you are enrolled in a Medicare Advantage PFFS plan that includes a Medicare prescription drug benefit, or any Medicare Advantage coordinated care (HMO or PPO) plan, you will automatically be disenrolled from your respective plan if you choose to enroll in a Medicare Prescription Drug Plan.
• If you are enrolled in a Private Fee-for-Service plan (PFFS) that does not include Medicare prescription drug coverage, a Medicare Advantage Medicare Savings Account (MSA) plan or an 1876 Cost Plan, you will not be automatically disenrolled from your respective plan if you choose to enroll in a Medicare Prescription Drug Plan.
