Medicare Special Needs Plans (SNPs)

Medicare Special Needs Plans (SNP) are  types of Medicare Advantage Plan designed for people with certain chronic diseases and conditions or who have specialized needs (such as people who have both Medicare and Medicaid, or people who live in certain institutions).

Medicare Special Needs Plans provide their members with all Medicare Part A (Hospital Insurance), Medicare Part B (Medical Insurance) services, and Medicare prescription drug coverage (Part D). Medicare Special Needs Plans were created to give certain groups of people better access to Medicare managed care.

Universal American offers two Special Needs Plans:

  • Tribute® is designed to meet the needs of people living in skilled/long-term care facilities across 10 counties in South Central Oklahoma. SelectCare of Oklahoma is owned and operated by Heritage Health Systems, a Medicare Managed Care and Medical Management Organization and a subsidiary of Universal American.
  • The Fresenius Medicare Care Health Plan (PFFS and PPO) “FMCHP” - through our relationship with Fresenius Medical Care North America, the world's largest, integrated provider of dialysis products and services, we are able to help improve the quality of life and clinical outcomes for Medicare beneficiaries with End-Stage Renal Disease through a  Medicare Special Needs Plan exclusive to beneficiaries on dialysis. Currently, the program is offered in Connecticut, Massachusetts, New York, Rhode Island, Illinois, Alabama, Tennessee and California. Starting in 2010, the plans will be also offered in the Metro Ft. Wayne, Indiana area as a Preferred Provider Organization (PPO) plan.  Learn more about 2010 plans.
