How to Enroll in a 2012 Medicare Advantage Plan
You can enroll in one of three ways:
- Enroll online using a secure
Medicare Advantage enrollment form.
- Call us at
1-866-249-8668, 8:00 a.m. to 8:00 p.m., in your local time zone (TTY users call
1-800-975-8089) 7 days a week, to enroll over the phone.
- Download a printable enrollment
form.
- Request the help of a licensed insurance specialist.
Before you enroll:
- Read the enrollment disclaimer information.
- Have your Medicare card with you.
- If you plan to enroll through a paper enrollment form and want to have your premium automatically deducted from your bank account, have your checking account and routing number available.
- Have any additional medical or prescription insurance plan information, if applicable.
- Have your Social Security information available.
- Choose your Primary Care Physician, if you are enrolling in a Medicare Advantage Health Maintenance Organization (HMO) plan.

Primary Care Physician (PCP)
A doctor that a member chooses to coordinate their healthcare with specialists and other providers. A PCP acts as the manager of your healthcare and can assist you in finding the right specialists when necessary. He or she is available to patients 7 days a week, 24 hours a day through regularly scheduled appointments or by other doctors providing "on-call" back up coverage. Most Health Maintenance Organization (HMO) plans require you to see your PCP before you see any other healthcare provider.
