Ready to enroll?

Before your loved one enrolls, make sure you have the right information before you start the enrollment application. You’ll need:

• The name of the plan the member wants to enroll in
• Member's first, middle and last name (spelled correctly) and date of birth
• Member’s home phone number including area code and permanent street address
• The mailing address to which all correspondence will be sent (this could be yours or the member’s)
• Emergency contact information (this could be you or another person)
• Member’s Medicare card
• Any additional prescription plan information, if applicable
• Social Security or checking account number and routing number if premiums will be deducted directly from an account and only if you are filling out a paper enrollment application to mail to us.
