What to Know Before Enrolling in a Medicare Advantage Plan

Make sure you have the right information before you start the enrollment application. You’ll need:

• The name of the plan in which you want to enroll
• Emergency contact information
• Your Medicare card
• Any additional medical or prescription insurance plan information, if applicable
• Social Security information or checking account number and routing number if premiums will be automatically deducted directly from an account
• The name of your Primary Care Physician, if you are enrolling in a Medicare Advantage Health Maintenance Organization (HMO) or Preferred Provider Organization (PPO) plan
