What to Know Before You Enroll

Make sure you have the right information before you start the enrollment application. You’ll need:

• The name of the plan you want to enroll in
• Your first, middle and last name (spelled correctly)
• Your date of birth
• Your home phone number including area code
• Your permanent street address
• The mailing address to which all correspondence will be sent (this could be yours or your caregiver’s)
• Emergency contact information
• Your Medicare card
• Any additional medical or prescription insurance plan information, if applicable
• Social Security information or checking account number and routing number if premiums will be automatically deducted directly from an account
• The name of your Primary Care Physician, if you are enrolling in a Medicare Advantage HMO or PPO plan
