• Your physician’s first name and last name
• City and state
• The health plan in which you are enrolled or want to search
• Whether you are searching for a Primary Care Physician (PCP)
• The physician’s specialty area of practice
• The distance you want to cover in the search

Note: If you plan to enroll in an HMO plan, you will be asked to select a Primary Care Physician (PCP). If you do not, one will be assigned to you. You can use this “Find a Physician” tool to search PCPs in your area. PCPs are grouped into Local Physician Organizations (LPOs) and these LPOs determine which specialists are available to you. For HMO Plans, both the PCPs and the specialists’ LPOs are listed to help in your search.
