What is a SNP Plan?
A Medicare Special Needs Plan (SNP) is a type of Medicare Advantage plan that includes all of the benefits of Original Medicare plus the additional benefits of prescription drug coverage. This type of Medicare Advantage plan provides focused and specialized care for specific groups of people. Coverage benefits, provider choice and drug formularies are tailored to the needs of the specific groups they serve.
SNPs limit membership to individuals who:
Receive both Medicare and Medicaid (also referred to as “dual eligible”); or
Live in an institution (also known as a skilled nursing facility) or who require the same level of care as those who live in an institution; or
Have specific chronic conditions (like ESRD or diabetes)

You may be eligible to enroll in a Medicare Advantage plan during your Initial Coverage Election Period (ICEP), when you become eligible for Medicare Parts A and B. This is usually during the three months prior, the month of, and the three months after your 65th birthday month or your 25th month of disability.
