A Special Plan for Special Needs
SelectCare of Oklahoma offers a Medicare Advantage plan known as Tribute® HMO.
Tribute® HMO is a Medicare-approved Special Needs Plan (SNP) designed to meet the needs of people living in skilled/long-term care facilities across 10 counties in South Central Oklahoma. SelectCare of Oklahoma is owned and operated by Heritage Health Systems, a Medicare Managed Care and Medical Management Organization, a member of the Universal American family of Companies.*
SelectCare of Oklahoma’s goal is to create a nursing facility-based medical program that provides members more care, more often, right where they live.
Each Tribute member receives routine evaluations and treatment from a Personal Care Team including a Primary Care Physician, and Nurse Practitioner or Physician’s Assistant.
Tribute is available in the following counties:
- Carter County
- Cleveland County
- Garvin County
- Jefferson County
- Johnston County
- Love County
- McClain County
- Murray County
- Oklahoma County
- Stephens County
* Our plan is available to anyone enrolled in Part A and Part B of Medicare through age or disability who continues to pay any applicable Medicare premiums and lives in the plans service area. You must continue to pay your Medicare Part B premium. SelectCare of Oklahoma and its agents are not connected in any way with the federal or state government or Medicare.
